Rating supplementary application form

I have a date for my psychometric test on the 24th and I'm not to worried about.

What is slightly bothering me is this Rating supplementary Form. I haven't played sports in about 10 years because of family situations getting in the way.

I'm a little worried how this will reflect on me (potentially in the interview), even though I was involved in sports a lot as a teenager will this make people think I'm not a team player potentially?

Just looking for opinions/experience on this matter.

It matters not. We're looking for team players rather than individualists, so currency and circumstances don't really matter, the fact is you've played team sports.

Teamwork can also involve work, education and suchlike, it doesn't have to involve current sporting involvement.
