Sending off my Medical Questionnaire

Good morning all.

I passed my interview last week and booked myself in for my eye test for the beginning of February.
Am I able to send off my medical questionnaire before my eye test and results?
Do I send it off with the EyecarePlan Voucher number which I have received?
I have completed my medical questionnaire which I am all ready for it to be sent off.

I'm very eager to do as much as I can to speed up and help with the process haha.

I sent off my medical questionnaire before having my eye test. Capita will still look over your medical questionnaire without the eye test results it will just mean you will be made TMU until they receive them.
